Abstract  This clinical pathway was developed by a team of nurses and physicians at a large home care agency (part of an integrated system of hospitals, long-term and subacute care facilities, and outpatient settings) for treatment of DVT with LMWH (low molecular weight heparin) in both the subacute and home care settings. Guidelines for diagnosis, patient selection for home care (exclusion from treatment) and a patient education plan(with flowsheet) were also developed with additional input from pharmacists and home care nurses. The pathway anticipates a 3 day or less length of intervention with documentation of outcomes, focused areas of assessment, treatment, activity, medications, tests and patient education. Since many patients with DVT can now be treated entirely at home or in an outpatient setting, use of a pathway to define expected outcomes of care and direction for patient assessment, care, monitoring and documentation is of great value.
